Athletic Registration:
Athletic registration for Berlin High School Athletic Teams takes place through the services of our partner company "FamilyID". Paper "Permission Forms" and "Emergency Information Forms" are no longer used or accepted. Instead, registration occurs online via https://www.familyid.com/berlin-high-school-athletics, which keeps track of the information in your FamilyID profile. Information is entered only once for multiple uses, multiple family members, and multiple programs throughout the duration of your high school athletic career.
INFORMATION NEEDED TO REGISTER
It will be helpful to have the following information handy to allow for accurate completion of your online registration:
Health Insurance Information, Physicians name and number, name and phone number of emergency contact.
A parent/guardian should register by visiting Berlin High School Family ID page and following these steps:
1. Under the "Programs" title, click on the link for the program you want to sign up for. Scroll to the bottom of the page to either Sign Up as a Family or Individual if this is your first time using FamilyID, or Log In if you already have a FamilyID account.
2. Sign Up for your secure FamilyID account by entering your family name, email address and password. You will receive an email with a link to confirm your new account. (If you don't see the email, check your spam or junk folders.)
3. Click on the link in your email confirmation, which will log you in to FamilyID, and then complete the information requested on the registration form. When you have completed the form, click the "Save & Continue" button.
4. Review your registration summary and click "Confirm Registration" to submit. Once completed, you will receive an email receipt from [email protected]. You can also view your completed registration on your Dashboard. You can log into https://www.familyid.com at any time to update your information and to check your registration(s).

STUDENT-ATHLETE HANDBOOK NEW 2020-21
This document outlines the expectations for Student Athlete behavior and conduct on and off the field of Play. Eligibility rules and Athletic Department Policies are detailed.
REQUEST TO ATTEND A TRYOUT/CLINIC/TOURNAMENT DURING THE SEASON
An athlete must have special Permission to Participate in an outside school event in the same branch of athletics during the authorized contest season or be rendered ineligible for all interscholastic athletics.
HEALTH FORM
A current State of Connecticut Health form is required to be on file in the Nurse's Office before a student may Participate in any athletic activity.
Physical Exams:
Physical exams must still be submitted to the Berlin High School Nurse's office prior to any participation. A physical exam must be current (within the past 13 months) and signed by the physician. Students will receive aBlue Cardor a Red Card, which they will hand directly to the coach on the first day of practice. If a student athlete obtains a Blue Card from the nurse the student athlete has been medically cleared to participate in their sport for that season. If a student athlete obtains a Red Card from the nurse the student athlete will have a physical which expires during their season and will need to update their physical. If the physical examination expires during the course of the sports season, the athlete will be required to be examined prior to the date of the physical examination's expiration, otherwise continued participation is prohibited. Student athletes will be required to obtain a blue or red card prior to each season (fall/ winter/ spring). If their physicals are good for the year, they must still go to the nurse’s office during the school year and receive a blue or red card to hand to their winter or spring coach for that season.
Students can pick up their Blue or Red Card from the nurse and/or drop off their physicals to obtain a Blue or Red Card. Dates and times are located on the athletics main page.
In the event that a card cannot be picked up on the noted dates/times, please email Leah Carbonell ([email protected]) to request one (a current physical must be on file for her to be able to do this).
ALTERNATIVE TRANSPORTATION FORM
A Parent must give their athlete written Permission to take an alternative means of transportation other than the team bus.
CAPTAIN CANDIDACY FORM
Any athlete may fill out this Captain's form at the end of the school year in order to be considered as a Captain on a team the following year.
PERMISSION TO PARTICIPATE
Parents and Athletes must sign this Permission form and give it to the coach when they sign up to Play.
EMERGENCY MEDICAL FORM
The coach keeps a copy of this form in the medical kit at all times in case of emergency.
IMPACT TESTING FORM
Athletes are given a concussion (IMPACT®) test by the Athletic Trainer at the beginning of their season in ninth grade and then again as Juniors. This Permission form should be signed by the Parent/guardian and given to the trainer so that the concussion testing can be done.
